Acadia Center for Nursing & Rehabilitation in Riverhead provides subacute care, serving patients with complex care and rehabilitation needs. It is a comprehensive program for someone with an acute illness, injury, or disease complication. This type of care is goal-oriented and applied after or instead of hospitalization. While this type of care is more intensive than traditional nursing facility care, it is less intensive than acute care.
