Several Clinical Services
Acadia Center for Nursing & Rehabilitation has a team of physicians, registered nurses, and physical, occupational, and speech therapists, along with consulting specialists and dietitians in a 24- Hour skilled environment. We offer a variety of clinical services, but our main focus is on the four areas below.
Subacute Care
Acadia Center for Nursing & Rehabilitation in Riverhead provides subacute care, serving patients with complex care and rehabilitation needs. It is a comprehensive program for someone with an acute illness, injury, or disease complication. This type of care is goal-oriented and applied after or instead of hospitalization. While this type of care is more intensive than traditional nursing facility care, it is less intensive than acute care. From IV therapy to pain management, respiratory care, wheelchair mobility care, and many other services, subacute care will stabilize many conditions and get patients back to their normal, daily routine as soon as possible.
Restorative Therapy
Our facility in the East End also offers restorative therapy 7 days a week for patients who have lost certain physical functions from a long hospital stay or as a way to provide rehab they need after surgery. The goal is to restore all the functions they would normally have, from walking to fine motor skills so they can return to their everyday life. Using a wide array of modalities we customize care to the specific needs of each patient. Restorative therapy focuses on achievable goals and the patient’s current abilities rather than their limitations.
Wound care
When you have a wound that is difficult to heal or becomes an ongoing concern as a result of diabetic, vascular, stasis, and other non-healing, chronic conditions, it’s important to receive specialized, professional care. We are committed to excellence, controlling infection and promoting healing. Our wound care team, which consist of a physician, a wound care nurse and physical therapist, evaluate your condition and set an individualized treatment plan for each resident. Just some of the technology we use includes wound-vac, pressure relieving beds, and stem therapy. We also work with patients recovering from graft procedures.
Long Term Care
Long term care at our facilities includes 24-hour nursing care, delivered with compassion by a caring staff. Our atmosphere is designed to make residents feel at home, and we welcome families at any time—our door is always open. Residents will enjoy meals in our spacious dining room. They can also participate in creative and therapeutic programs that meet their interests. Private and semi-private rooms are available, and are complete with attractive bedding and window treatments. Every room has cable television, WiFi, and phone service available.
